Complete and submit this form by 5:00 p.m. ET on December 1, 2017.  Incomplete proposals will not be accepted.

2018 Summit Process Timeline:
- All submissions received by December 1, 2017
- Notification of Committee selections week of January 17, 2018
- Review/edits of abstracts and bios on website by January 31, 2018
- Initial slides due for Committee review April 13, 2018
- Rehearsals complete April 20, 2018
- Final slides due for legal review April 27, 2018
